Student health significantly influences one’s ability to participate fully in the School community. Our school nursing staff at Spruance strives to enhance each student’s well being by insuring that each student has a complete medical history including up-to-date emergency contact forms. We believe that good student health begins with prevention and we regularly screen students for vision and hearing services in accordance with Pennsylvania state regulations.

We provide care to students with chronic illnesses and minor injuries. We’re your partners in supporting your child’s health.

REQUIRED MEDICAL FORMS FOR SCHOOL ENTRY

 Does your child have a health condition we need to know about?

As the school nurse at Gilbert Spruance, I would like to know about your child’s health condition, so I can help you and your child have a successful school year.  Please complete the form (S-865) and return it to the nurse as soon as possible.

If your child requires medication in school, please contact the nurse or send your child to the health room for the proper form (MED-1) to be completed by your child’s doctor.  For everyone’s safety, student’s ARE NOT permitted to bring any medications into school without written permission from the parent, doctor, and the approval of the school nurse.  This includes asthma inhalers and over the counter medications.

Does your child need medication during school?

For health problems that may require medication administration in school, the required MED-1 form is required and MUST BE COMPLETED BY A DOCTOR.

For everyone’s safety, student’s ARE NOT permitted to bring ANY medications into school without written permission from the parent,  instructions from the doctor, AND the approval of the school nurse.  This includes asthma inhalers and over the counter medications.
